Health Check Physical Therapy accepts payment from the following medical insurance companies:

Additionally, Health Check Physical Therapy strongly suggests that you contact your medical insurance company prior to your first scheduled appointment. We do not want you to be surprised by unexpected charges including, but not limited to, co-insurance and co-pays.

Should you have medical insurance from a company not listed above, don’t let that keep you from receiving the treatment you need! Instead, contact Health Check Physical Therapy as suggested below.

Other Payment Options

Health Check Physical Therapy also accepts payment from open and active claims covered by medical benefits arising under and covered by Workers Compensation.  At its discretion Health Check Physical Therapy may accept payment resulting from Personal Injury cases.

Health Check Physical Therapy also accepts MasterCard, VISA and Discover cards as well as cash.

Finally, it is important to understand that Medicare, Workers Compensation and motor vehicle insurance do not pay for treatment under the Direct Access Physical Therapy option. Consequently, interested patients must consider other payment options.
